What is sorbitol intolerance?

Sorbitol, also known as E420, is a sugar substitute that is considered healthier than regular industrial sugar. In a healthy person, the sugar alcohol sorbitol is absorbed through the intestinal membrane into the blood and then broken down by the liver. In the case of an intolerance to sorbitol, digestion in the small intestine is not functioning properly or is restricted. Thus, the sorbitol reaches the large intestine, where certain bacteria carry out further digestion. The decomposition products are short-chain fatty acids, the breathing gases hydrogen, carbon dioxide and methane. The latter causes the gut to swell and the typical symptoms to appear: diarrhoea, flatulence and constipation. Larger amounts of sorbitol cause digestive problems even in people with a healthy gut.

People with a fructose malabsorption disorder should also avoid products and foods containing sorbitol, as it is suspected of inhibiting the GLUT transporters and thus worsening the already limited fructose absorption.
